At the Main Panel you have the 2 hot wires and the grounded neutral. In the main panel, the white and the ground wires are bonded to the cabinet on the same bus bar. From here you would feed the 4 wires to the spa GFCI sub-panel. (This means that in the main panel, the white (neutral) and green (ground) are on the same bar. In the spa sub-panel you would have the ground wire terminate at the ground bus bar (which is bonded to this cabinet. The white (neutral) wire terminates on a separate bus bar which is iunsulated from this cabinet. Then from the spa GFCI disconnect you have your 4 wire set-up (the 2 hots, insulated neutral, and bonded ground).
